This survey is for parents and carers whose children are aged 0-25 years with Special Educational Needs and Disabilities (SEND) and who live in Wolverhampton, or access Wolverhampton Services.
If you would like help to complete this survey please contact Voice4Parents 01902 550396 v4p@wolverhampton.gov.uk and a member of the forum will be happy to assist you.

 What is the survey about?
Voice4Parents is a group of parents and carers who represent the wider parental voice to make positive changes to the local SEND services families receive.  We want to ensure as many parents and carers have the opportunity to share their views as possible.
You can do this is by completing this anonymous survey.
Voice4Parents has been involved in co-producing many aspects of special educational needs and disability practice in Wolverhampton.  It is imperative that we remain active partners in reviewing how new systems are working.  Your experiences will enable us to provide constructive challenges to the local authority and other services about what is working well and where improvements can be made. The survey will also be used to gather parental views during local area SEND inspections.  If you would be willing to speak with inspectors directly about your personal experiences then please contact Voice4Parents.
The survey should take no more than 10 minutes to complete and the information you give us is confidential and anonymous.
If you have more than one child with special educational needs and/or disabilities please complete a survey for each.
