We are a team of researchers at University College London who are interested in understanding how women aged 25 to 64 take care of their health. The answers you give here will help us to learn about women like you. At the end we will give you feedback on your answers, and how they compare to a healthy lifestyle.
As a thank you for your time, at the end of the survey you will be invited to enter your email address for a chance to win £50. If you don't provide your email address you won't be entered into the draw as we will have no way of knowing who you are.
All your answers will be kept strictly confidential. If you decide you don't want to continue with the survey you can close your browser tab or window and all of your answers will be deleted.
