Welcome to the Synergi Collaborative Centre's National Public Survey on Ethnic Inequalities and Severe Mental Illness.

We would appreciate your assistance in helping us to identify the top priorities for research on ethnic inequalities in severe mental illness in the UK. This is an important national survey and is the first of its kind, helping us to shape the future research agenda, and to involve those with lived experience of mental illness and the public, as well as scientists and clinicians. Please have your say.

Your answers to the survey questions will not only shape Synergi's future programme of work, but will help us tackle mental health inequalities and to campaign to transform health systems.

In this survey you will be presented with a number of subject areas that we have identified from existing research, service users, clinicians and other stakeholders. You can also enter free text comments at the end of the survey.

For each issue identified, we would like you to rate its importance on a scale from 'not important' to 'vital' in relation to the following question:

What are the most important issues that we should understand when trying to reduce ethnic inequalities in severe mental illness?

Your answers are confidential and will be aggregated with all respondents to show us the overall opinions and which issues are rated as the most important.

No identifiable personal data will be disclosed to any third party. All data will be securely stored on a password-restricted computer.

The survey will take an estimated 15 minutes to complete. Once again, thank you for taking the time to fill out this survey.
