What this survey is about

This survey has been compiled by Sheffield Parent Carer Forum with funding from the government's Department for Education.

It is aimed at parents and carers of children and young people aged 0-25 years who have special educational needs and/or disabilities (SEND) and who live in or access services in Sheffield.

The questionnaire covers all aspects of caring for a child with SEND - there are sections on family life, work and finances, childcare, education, social care, healthcare and transition. You can skip any sections that are not relevant to you. It takes around 15 to 20 minutes to complete. You don't have to do it all in one go - you can close the survey down and come back to it the next day. Your previous answers will be saved. The deadline for responses is Friday 18th January 2019.

Your feedback is very important, because it provides an evidence base that can be used to improve services for our children. We have already shared preliminary findings from this survey with Ofsted/CQC inspectors when they visited Sheffield in November 2018 - this was very useful! We will publish a final report in February 2019, and we will work with decision-makers in Education, Health and Social Care to ensure that your feedback is used to make improvements.

At the end of the survey, you will see a code which you can email to us to enter a prize draw. There are some great prizes to be won, including shopping vouchers worth £50, £30 and £20!
