Thank you for agreeing to take part in this survey.

It is our hope that as many children, young people, parents, carers, staff members and support agencies will have the chance to have a voice and share their experience of children and young people’s mental health and wellbeing services in Swansea. The results will be important in informing how Swansea’s Third Sector plans and delivers mental health and wellbeing services for children and young people but will also prove valuable to the Local Authority, ABMU Health Board and the 3 GP Cluster Networks.

 

The survey is in 3 parts:

 

PART 1 of the survey will help us get a good idea of:

-         what is working well

-         what isn’t working well

-         what would have made people’s experiences better

-         what services might look like in the future

-         what support you/your organisation might need to better meet the mental health and wellbeing needs of children and young people.

 

PART 2 of the survey asks for specific data on the number of children and young people you have supported in relation to mental health and emotional wellbeing between 1st January 2017 and 31st December 2017

N.B. Please only complete once per project or organisation

 

PART 3 of the survey asks questions about adverse childhood experiences (ACEs) and other traumatic experiences that occur before the age of 18.  These experiences range from suffering verbal, mental, sexual and physical abuse, to being raised in a household where domestic violence, alcohol abuse, parental separation or drug abuse is present.
