Social Prescribing Services Survey

Introduction
Thank you for participating in this survey. The purpose of this survey is to gather information about your experience with social prescribing services specifically related to mental health support. Your responses will contribute to our understanding of how residents perceive the social prescribing service within the borough. The survey should take approximately 15 minutes to complete. All responses will be kept confidential and used for research purposes only. Your participation is voluntary, and you can choose to withdraw at any time. Please answer the following questions based on your personal experiences.
 
What is social prescribing?
Social prescribing enables a GP, nurse or other healthcare professional to refer you to a specialist link worker to talk about the issues you are facing and find the right services to support you.

Link workers give people time, focusing on ‘what matters to me’ and taking a holistic approach to people’s health and wellbeing. They connect people to community groups and statutory services for practical and emotional support.
 
Social prescribing works for a wide range of people, including people:
  • with one or more long-term conditions
  • who need support with their mental health
  • who are lonely or isolated
  • who have complex social needs which affect their wellbeing, i.e. money and debt issues, domestic abuse, employment
The service helps you improve your health and wellbeing and to make positive life change.
