Together, we can help people experiencing depression.

We’re using our lived experience of depression to start a new social enterprise: Reconnect Wellness Club. If you live in the UK and have experienced depression, we want YOU to join us on this journey by sharing your experiences.

This survey will use the term depression to describe depression, anxiety and related conditions. We understand people are often given a range of different terms to describe their suffering, however, for this survey we will use depression to encompass all of these terms.

Absolutely all responses are anonymous. We won't ask you for any identifiable information in this survey.

This survey will last between 5-10 minutes.
