About the survey

We are conducting a research study at Hammersmith Hospital (Imperial College Healthcare NHS Trust), that hopes to gauge the feelings and beliefs of members of the public towards disease risk, disease prevention and genetic testing. We hope that the results of our study can be used in the future to improve the way in which we promote disease prevention to the general public and the ways in which members of the public are selected for screening programs for diseases such as cancer. The aim of this would be to hopefully allow cancers to be diagnosed earlier, particularly in those people who are at higher risk of developing cancer because of changes in their genes. In doing so we would hope to be able to treat people more quickly and successfully.

This questionnaire will explore your views on this subject and we would be very grateful if you could spare the time to complete it. This survey is aimed at people over the age of 18 years old who are registered with a GP and not because of any health issues that you may have.

We very much appreciate your time in completing this questionnaire which we hope will take no longer than 10-20 minutes of your time.

You will see a list of statements that are related to genetics and genetic testing. Please select the appropriate column as the statement pertains to you. Some questions will not apply to you. Please make a mark under “Does not apply to me” in these cases.
