Survey Description

I am a dietitian and researcher from King’s College London (KCL) and Guy’s and St Thomas’ NHS Foundation Trust.  As part of my PhD studies I am conducting a research project to improve the nutrition service provision of falls services. Before you decide to take part, I would like you to understand why the research is being carried out and what your participation will involve. Please read the information below.

In order to understand current practice, I am circulating this survey to explore current nutritional care practice and procedures in falls services across England. As a healthcare professional member of a falls service, I am inviting you to take part in this short survey (5-10 minutes) on behalf of your service.

Participation is completely voluntary and choosing not to take part will not disadvantage you in any way. If you choose to take part, you will be asked to confirm your consent in the question below.  You are free to withdraw at any point during completion of the survey by exiting the survey, without providing a reason. Withdrawing from the study will not affect you in any way. Once you submit the survey, it will no longer be possible to withdraw from the study. 

The survey is anonymous and no personal identifiable information will be collected. Your answers will be treated confidentially and the information you provide will not allow you to be identified in any way.  Data will be held securely within King’s College London for one year after completion of the study.

The results of this survey will be used for research purposes only as part of the PhD thesis (Massar Dabbous) and may be written up for scientific journals or healthcare conferences. The information you provide will not allow you or your organisation to be identified in any research outputs or publications.

 
The research has been registered with the King’s College London ethics office (ref #: MRS-18/19-14198). The data controller for this project will be King’s College London (KCL). Research is a task that the University carries out in the public interest. Your data will be processed in accordance with the standards set by the General Data Protection Regulation 2016 (GDPR).
