This survey is to support the NHS Clinical Commissioning Groups in Coventry and Warwickshire understand more about the experiences you have had of neuro rehabilitation, either as a patient, a friend, relative or carer of a patient or as someone who works with patients either within the voluntary sector or as a member of staff.

We are undertaking this survey as, before the start of the COVID-19 pandemic, the “Level 2b neuro-rehabilitation service” was located at University Hospitals of Coventry and Warwickshire. This service cares for patients who need post-acute, specialist rehabilitation following brain injury, complex neurological conditions or acute neuro-behavioural conditions, at a level that is less intensive than patients with the very highest acuity.

In order to help the NHS respond to COVID-19 a decision was taken on 18th March 2020 for the service to be moved from UHCW to the Central England Rehabilitation Unit (CERU), a dedicated rehabilitation facility which is part of Royal Leamington Spa Hospital, located on Heathcote Lane in Warwick.

The Clinical Commissioning Group is now considering the future location of the service. The information that you share will be used to inform any decisions made about where the service should be located in the future. The information provided will only be used for the purpose it has been collected for and will not be passed on to any third parties.
