Macmillan Community Cancer Nurse Service Questionnaire

Thank you for your interest in helping us to develop this service. It’s vital that we understand what patients and carers need from the Community Cancer Nurse team and we appreciate you taking the time to let us know what you think. We’re keen to understand what is currently working well but also where the gaps are; your experience will help us to recognise these and make the new service effective for the people it serves.

- Patient recently diagnosed and about to start treatment

- Patient currently receiving treatment

- Patient has completed treatment pathway and is being supported by community services
