Matchday Experience Evaluation Form

Matchday feedback helps CAFE to ensure we have the most up-to-date and accurate information on access and facilities at stadiums across Europe, but it also helps disabled fans who may be planning on attending matches at the stadium to prepare in advance.

Matchday feedback can also lead to access and inclusion improvements at stadiums. 

This questionnaire is designed to follow the matchday journey of a disabled supporter, from arriving at the stadium to the view of the pitch from the accessible viewing areas. 

It also asks questions on any accessible services, amenities and facilities that a disabled fan may use during the match.

We would like to hear from a variety of disabled people who attend live football matches at stadia across Europe. If you are answering this questionnaire on behalf of a disabled person you support, please provide us with his / her information.

The survey comprises of four sections, and should take approximately 10 - 15 minutes to complete. Please only answer questions that apply to your own matchday experience at the stadium - you do not need to answer every question. We would appreciate it if you could give as much detail as is possible on all questions that you do answer.

Saving your answers:

If you are answering the survey from an email link, you can start the survey, exit and come back to it another time using the same link. Your answers will be saved by clicking on 'Next' or 'Done' at the bottom of each page. If you click the exit button, responses entered on the current page will not be saved.

If you are answering the survey via a web link, responses are saved through cookies. If you start the survey and return to it later using the same computer and browser, you can pick up where you left off. However, if you disable or clear your cookies after starting the survey, your answers will not be saved, should you decide to return to the survey later. 

Data Protection:

Your participation is entirely voluntary, and all responses and will be regarded as strictly confidential. No data will be accessed by anyone other than staff at CAFE.

CAFE may use some of the questionnaire responses in future publications, but no comments that we publish will be attributable to respondents and names will not be published. We may contact you to discuss your experiences in further detail, and any contact details you provide will never be shared with any third parties.
