Please complete this form as soon as you can after the match you have attended, whilst your recollection of the matchday experience is still clear.

User-led feedback from disabled supporters and their companions attending live matches is vital in reaching #TotalAccess.

CAFE proactively seeks feedback on matchday experiences, including what was good, what was poor and any suggestions for improvements. This enables us to work with our stakeholders to improve access and inclusion at stadiums and make the matchday experience a more welcoming and inclusive one for all disabled spectators.

Matchday experience from disabled supporters can also help CAFE to ensure we have the most accurate information on stadia which can help provide fellow disabled supporters plan their journeys to stadiums that they may not be familiar with.

This questionnaire is designed to follow your journey from arriving at the stadium to reaching your allocated seat. It also asks questions on any accessible services and facilities you might use during the match and on the customer service you receive from staff and volunteers.

Please only answer questions that apply to your own matchday experience, you do not need to answer every question. Please also provide as much detail as possible.
