Please complete this questionnaire as soon as possible after the match, whilst your recollection of the matchday experience is still clear.

Detailed user-led matchday feedback, from disabled supporters and their companions, is vital in promoting Total Football #TotalAccess.

CAFE proactively seeks feedback on matchday experiences, including what was good, what was poor and what could be better. This helps us to work with our stakeholders to improve access and inclusion at stadiums, and make the matchday experience a more welcoming and inclusive one for all disabled spectators.

Matchday experience feedback also helps CAFE to ensure we have the most accurate stadium information on our website, helping other disabled fans to plan any future visits to the stadium.

This questionnaire is designed to follow your matchday journey, from leaving home and arriving at the stadium to reaching your allocated seat, watching the match and leaving the venue.

It also contains questions on any accessible services and facilities you may have used during the match and the service you received from stadium staff and volunteers.

Please only answer questions that apply to your own matchday experience - you do not need to answer every question. Please also provide as much detail as possible to best illustrate your experience.

If you are answering this questionnaire on behalf of a disabled fan, please provide us with their contact information.

CAFE would welcome receiving photographs from your matchday which may illustrate areas of best practice or areas of concern. Please email any photographs you wish to share to CAFE's Fan Liaison, Access and Administration Officer, Amy Wilson, at amy@cafefootball.eu.

If you require any assistance to complete the questionnaire, or would like a hard copy, please contact Amy by email to amy@cafefootball.eu or call +44 (0) 208 065 5108.

CAFE would like to thank you for your participation. Your feedback is vital in working towards a game that is accessible, inclusive and welcoming for all.
