Welcome to the stakeholder engagement survey for Joint Strategic Needs Assessment (JSNA): Dementia. A JSNA is an area of work that looks at the current and future health needs of the local population to inform and guide the planning of health, wellbeing and social care services within Wolverhampton.
As part of the JSNA, Wolverhampton City Council is conducting a study on services supporting residents with Dementia. An important part of the study is to understand what services are currently being used, peoples experiences of these services and highlight the unmet needs of residents with Dementia. The outcomes will be used by the JSNA Steering Group to recommend ways in which services can be improved. Your response will help us to build a picture of the services currently used and work towards improving them.
We would like to invite you to complete this survey if you care for people diagnosed with Dementia living in Wolverhampton. It should take approximately 10 minutes to complete the survey. All responses from the survey will be analysed together and overall findings will be presented in the final report to the JSNA Steering Group. We can also provide a summary of findings from the survey upon request. All survey responses will be kept confidential and no identifiable information will be collected.
