Carers
The practice is committed to supporting patients who act as formal or informal carers. We are running a campaign to update our register of patients who act as carers so we can ensure you are supported both by the practice medical team and offered help through local carer support agencies.

Patient Participation
The practice has an active patient participation group, Friends of Forum Health Centre, and we are keen to recruit new members to ensure our group is as representative as possible. The group meets bi-monthly and the next meeting is Monday 5th September 2016 at 6pm.

Social Prescribing

The practice is working alongside Coventry AgeUK to help patients who would benefit from extra non-medical support to improve their health and well-being. The Social Prescribing Service will help support patients in the community to: reduce isolation and loneliness; increase confidence; improve fitness and increase mobility; reduce stress and improve mood; claim benefits and maximise well-being; and identify goals, aims and agree an actions plan.
