Your experience

The NHS in this area is looking at the way people are cared for near the end of their lives.

We want to work with patients, carers, people who have been bereaved, and staff to develop a clear vision of what excellent care looks like, and what changes we must make to improve support for patients and carers.

This survey is the first step in that process - your views will help us to identify the areas we must focus on first.

We want to know how well you feel you were supported (as a patient, or a carer) throughout your experience of end of life care - please think about care given at all stages, from diagnosis onwards.
