Across Teesside (Middlesbrough, Redcar, Hartlepool and Stockton), there are four main Mental Health Crisis services (Crisis team, Street Triage, Crisis Assessment Suite (CAS) and Liaison Psychiatry).

In order to ensure that the design and delivery of Tees Crisis services across; Middlesbrough, Redcar, Stockton and Hartlepool, are informed by public opinion, this survey will ask about your general understanding of: 
1) Mental Health Illnesses and
2)Tees Mental Health Crisis services.

Everybody is asked to answer Q1 to Q10  after which you will be directed to answer either Q11 to Q21 or to go straight to Q21.

It will take approximately 5-7 minutes to complete the main survey. 

In addition there are also 16 equality and diversity questions at the end of the survey which we would like everybody to answer, but you do not have to answer all or any of them if you do not wish to disclose this information.  These should take no more than 5 minutes to complete.
