Welcome to our survey! Thank you for sharing your time and your views with us.
This survey can be answered by anyone in Leeds who has (or had) a mental health condition or cares for someone who does. You (or they) don’t need to have a formal diagnosis to take part.
The survey has no more than 10 questions (not including the monitoring information at the end) and it will take about 2 to 10 minutes, depending on how much you want to share.
We won’t ask you for any information which could identify you. Your views and experiences will be used to inform a report that will be shared with the people designing mental health services in Leeds and published on the Healthwatch Leeds website.
The survey is due to close on 30th November 2023.
