The survey allows parents to respond anonymously with an option of providing details should they want to.  

Each question allows for a grading of Agree to Disagree (1-5), with 5 being the highest level of agreement and 1 being the lowest level of agreement. Each question provides a box for parents to add comments.

The survey aims to assess perception of communication between parents and school but also encourages parents to provide evidence of their claims.
