The Care Crisis Review is looking at what might help to safely reduce the number of children coming into the care system. This survey will help to make sure that young people and adults who have been in care have the opportunity to contribute their views to the Review. Please read this short summary about the Review before completing this survey.

Please first read the information below about taking part in the survey:
  • Taking part is voluntary.
  • You may find that not all of the questions feel relevant to you. You can just answer those which are relevant and that you are happy to answer.
  • We will discuss the information gathered through this survey in a report about the review and in articles, presentations and interviews. Any names and places will be changed so you cannot be identified.
  • At the end of the survey, you can provide us with your name and contact details, but it is up to you whether you wish to provide these.
  • Those carrying out the Care Crisis Review comply with Family Rights Group's Confidentiality Policy and Privacy Policy.
