In the first section I ask for some details about you. We've kept this to a minimum to save you time, and it will really help us to build better understanding of the overall situation.
If you'd like a copy of the report that we will produce as a result of this research, please include your name and email address. This will be treated in the strictest confidence and used only to send you the report.
And, of course, there will be nothing in our final report to identify any participant who has taken part in this research.
