Listening to your experiences of end of life care

NHS Dorset Clinical Commissioning Group (CCG) is inviting people who have had experience of end of life care over the last three years to come forward and give their opinions. These views will help shape the future of Palliative Care in the Community services across Dorset.

Over July 2021 we will be running a survey and having conversations with people who have experience of end of life services.

The importance of understanding what matters to patients and families who have experience of end of life care is key as part of the planning process for this service.

If you would like to share your experiences with us, please register your details below. A member of our team will be in contact soon for an informal conversation, so we can take the time to listen to your story.

We collect and store your data in line with data protection regulations. Your information will only be used for this project. We will not share your information with other organisations. If, at any point, you want us to remove your details from our database, or if you have any other questions about this work, please contact communications@dorsetccg.nhs.uk
