Thank you for joining the Arts 4 Dementia Best Practice Conference at the Wellcome Collection. Your contribution to our campaign for social prescribing on diagnosis of a dementia will help preserve life in the community for the 225,000 people diagnosed each year.

We shall report on developments for publication next year to demonstrate how primary care networks are beginning to offer social prescribing for dementia and what new volunteer creative befriending services for individuals on diagnosis of a dementia are on offer. To this end we shall be sending out three surveys to record new developments.
