This application form is for young people with alopecia aged 13-21 living in the UK who wish to be a part of the Alopecia UK Youth Voice Board.
If you are age 17 and under you will require a parent or guardian to read through this application with you and provide their consent.
The application consists of 4 sections:
1. Tell us about you
2. The Youth Voice Board Agreement
3. Consent
4. Equality and Diversity form (optional)
Once your application has been submitted someone from Alopecia UK will contact you to confirm that it has been received and provide details on any next steps. If you are aged 17 and under we will also contact your parent/guardian to share with them a copy of your application.
