Emotional Wellbeing of Children and Young People Survey
The Healthwatch Lewisham Youth Board is a group of volunteers who are between the ages of 14-21, their aim is to give children and young people a voice in health and social care services and to help service providers to understand what is going well and what can be improved.
This survey has been co-produced with the Youth Board to understand the emotional wellbeing needs of children and young people in Lewisham and your preferred ways of accessing support.
We are also interested in hearing about your experiences of using emotional wellbeing services such as Kooth.
As a young person, we invite you to take part in this survey, it will only take a few minutes of your time and all answers you give will be kept anonymous and confidential.
Please note that this survey is for children and young people who live in Lewisham and are between the ages of 11-19, or up to the age of 25 if you have a learning impairment/difficulty or are a care leaver.
Terms and Conditions of Survey Prize Draw
By filling in this survey, Healthwatch Lewisham is offering participants the chance to win a £50 voucher. One person will be randomly selected to win the prize. Only completed entries will be entered into the draw. To be eligible, you must be a Lewisham resident. Vouchers are not exchangeable for cash. The winners will be contacted following the draw. Winners will not be required to participate in any publicity.
