Introduction

People and Work, a charity based in Cardiff, has been commissioned by the Welsh Government to evaluate the new autism strategy for Wales.

You can find out more about People and Work at http://peopleandwork.org.uk/en/home/

You can find more information about the Welsh Government’s autism strategy at http://gov.wales/topics/health/socialcare/asd/?lang=en

As part of the evaluation, People and Work want to find out more about the experiences of families accessing the new Integrated Autism Service. This questionnaire has been developed to enable people to share their experiences.

We will use the information people provide about their experiences to help produce a report for the Welsh Government on the difference the new autism strategy is making to people like you in Wales.  We don’t ask for your name and your response will be anonymous.

Although we would very much like you to find out about your experiences, it is your choice and it will not cause you any problems if you choose not to complete this questionnaire.

If you have any questions, please email admin@peopleandwork.org.uk or telephone 02920 488 536
Please read each question carefully and answer the questions by clicking the button next to the answer. You can choose not to answer any questions.
