Neath Hill Health Centre Patient Survey

NHS England and Milton Keynes Clinical Commissioning Group are the NHS Organisations that plan, pay for and monitor the quality of local GP Services.

We are committed to GP services continuing for all the patients who currently use Broughton Gate Health Centre. We will be setting up a new contract for the service and invite organisations, including the current provider of GP services at Broughton Gate Surgery, to bid to run the service.

As part of the process, we are keen to find out more about what local patients want from their doctors surgery. Your views will also help us to shape GP services across Milton Keynes in the future.

The survey will close at 5pm on Sunday 6 November 2016 and your responses will be kept confidential and anonymous.
