This is a study run by the Faculty for Public Health UK and Eating Better.
 
Before you decide to take part in this study, please take time to read the following information. Contact us if there is anything that is not clear or if you would like more information.
 
This survey aims to gather information about the position and practices of UK health organisations related to sustainable diets. Following the end of the survey we will publish the results and this may include quotes of comments which will be anonymous. you agree to participate, you will be asked a couple of questions about your organisation and the general nature of your role within it; no other identifiable information will be asked. This survey should take no longer than 10 minutes, and participation is entirely voluntary. You have the right to withdraw at any time by closing your browser, up until the time of submission.
