Health Needs Assessment

We are taking part in the regional Better Health at Work Award scheme aimed at improving the health, safety and wellbeing of the workforce. Part of this work will involve looking at health training, work based health interventions, policies and our culture around wellbeing at work. It will also include displays, posters and other campaigns and events taking place. This scheme is coordinated by the Northern TUC and more details may be found at www.betterhealthatworkne.org

Employees, who will be supported in helping to organise these campaigns are called Health Advocates and part of their role is to find out what health issues are important to you and ways in which you would like to receive information and/or participate in health improvement activities. We are looking to recruit Health Advocates and if you are interested in supporting some work of this nature, however little your contribution, please contact _______Sophie Cook___.

By completing the following questionnaire, you will help us and future Health Advocates to address the health issues that are important to you. The questionnaire should take no more than 5 minutes to complete.

This Health Needs Assessment is entirely confidential and we encourage you to be as honest and open as you can be. You will not be asked to enter your name or any personal identifiable information and none of the questions are compulsory.
