We would like to invite you to complete this survey as part of the Walking Works project. The purpose of this survey is find out about how you travel, your physical activity and what you think about walking.

It will only take 10-15 minutes to complete.

All information that you provide is strictly confidential and will only be seen by the evaluation team at Loughborough University. No personal data will be made available to your employer and your individual details will not be used when the study is written up or discussed in the future.

Data will be collected, stored and used in accordance with the Data Protection Act 1998 and only authorised personnel will have access to it.

Please answer as many questions as you can and remember there are no right or wrong answers. Your participation in this survey is voluntary and you are free to withdraw at any time.
