What kinds of service you offer (e.g. group transport, dial-a-ride etc)

How many 9-16 capacity minibuses you operate using s19 / S10B permit

How many vehicles you operate using s22 permit

How many individual passengers you serve (e.g. total on your membership list if you have that system - estimate will do.)

Any breakdown of the above by characteristics (e.g. % older people, % learning disabilities, % sensory impairment etc)

How many single passenger trips you deliver per year (estimate will do) – these need to separate s19 trips and s22 trips

How many community organisations you provide group transport for (e.g. group membership if you have a membership system – estimate will do)

Apart from core CT services such as group transport or door to door accessible transport, please detail any work you provide under contract for education (SEN) or social care or health care providers using s19 or s22 permits and how may vehicles regularly used on this?

Do you hold a PSV ‘O’ Licence?
