Introduction

Jersey's Health and Social Services Department is currently reviewing the way in which equipment for children with disabilites and health needs is provided.

 

We are doing this to make sure that the identification and supply of equipment and medical products to children and their families works well, meets children's needs and supports independence.

 

 

As part of the review, we want to hear from parents and carers of children and young people who have used, or currently use the service.




Please take just a few minutes to answer these short questions about your experiences to feed into the review. Questions 1 - 8 ask you about equipment,  questions 9 - 15 ask you about medical products and consumables and questions 16 - 21 ask you for other information. Your answers are anonymous
