Warwickshire County Council (WCC) Engagement: Community Provision for Working Age Adults with Disabilities and/or Mental Health Needs

We are looking at the support people have in the community across Warwickshire. This could be support at home, day service support, respite or the Learning Disability Hubs. 

Why are we asking you to fill in this questionnaire?


We would like to know what works well and what could be improved in services.This is to make sure the service meets everyone’s needs.
We are asking people who have this support some questions to find out more.
