Atrial Fibrillation [AF] and Diabetes

Quality of Life survey

This survey is for people who know they have AF and diabetes and is designed to understand more about how this may or may not, affect quality of life. There are a series of multiple-choice questions and this survey should take no more than ten minutes to complete.

The survey feedback will be evaluated and summarised as part of a PhD in Health Research, being undertaken by Angela Hall, an Arrhythmia Nurse Specialist based in Jersey and through the University of Lancaster.

The results will be compared to the same survey being completed by people with AF alone in an attempt to see whether quality of life differs between people with both AF and diabetes and people with AF alone.

By completing this survey, means consent has been given to use the answers towards overall results. These results will be used to write up journal papers for publishing, a report to be included in the PhD Thesis and for sharing at conference poster and / or oral presentations.

No personal or identifiable details are needed and this survey is therefore anonymous. Please do however note the memorable / ID number you enter in the last question. If you wish to withdraw your completed survey at a later date (before 31.12.2019), this will be needed so the researcher can locate your survey.
