This survey was created by South Tyneside Mental Health User Voice service, which is part of Mental Health Matters. We work with adults who are receiving support from mental health services and who want to share their experience to help improve services.

We would like to ask you some questions to gather feedback on adult mental health provision in South Tyneside. If you are receiving mental health support from either statutory or voluntary sector organisations, we would like to hear your views.

Any feedback provided will help us understand what is currently working well, what can be changed or improved and if there are any gaps in service provision. The information you provide will be compiled into a report to help improve service provision.
