Important note: please be aware that there will be mixed sleeping arrangements this year. If you have any concerns about this, please contact our Wim Co-ordinator, Rachael, at wim@rev.org.uk, so that an alternative can be organized.

* 1. The Basics

* 2. With which section of the choir do you intend to sing with at WiM?

* 3. Which Rev are you from?

* 4. Which days do you intend to be at WiM?

* 5. In which splinter groups might you be interested?

* 6. Which T-shirt size would you like?

* 7. Please give details of any instruments you play. (To what level? Do you intend to bring it with you?) (This year the Arts team are planing to have a fairly simple band eg. piano/guitar/drums)

* 8. Please give details of any dietary/medical requirements (vegetarian, vegan, allergies, diabetes, etc).

* 9. Please indicate the following:

  Yes  No
Have you been to WiM before?
Do you have a First Aid certificate?
Are you interested in teaching a song? (only is signing up before 11th March)
Would you like to opt out of Angels and Mortals?
(Each Revver is assigned a "Mortal", a bit like a Secret Santa. You are this person's "Angel" and generally do nice things for them throughout the week/buy treats etc).
Do you give us photo permission for general publicity?
Are you okay with strobe lighting being used at the concert?

* 10. Is there anything else you think the team should know about? (eg. medical conditions for us to be aware of). All Revelation national events now have a mixed sleeping policy as standard. If you might require alternative arrangements, please let us know at wim@rev.org.uk and we'll do our best to accommodate.
