Move More Eat Well Survey

Hi, thanks for agreeing to help the Move More Eat Well programme by answering a few questions about how active you are, how healthily you eat and then hopefully agreeing to be part of our exciting new programme!

The survey shouldn't take more than 10 minutes to complete, so it’s nice and short. Questions marked with * are mandatory unless otherwise stated.

And of course, your personal data will only be held by or on behalf of the Move More Eat Well programme and will not be shared with any other organisation.

 

Your completed surveys will go into our free quarterly prize draw to win a choice of £50 worth of Fruit and Veg or a £50 voucher to spend at 3CJ Cycle Centre in Longbridge. The full prize draw Terms & Conditions are attached available on our website here.

The Move More Eat Well programme will only gather and store your personal information (such as name, e-mail, address, mobile number and other relevant information related to our programme) after having been given permission by you to do so.

The insight we gain will be anonymised and shared with non-commercial organisations like Birmingham Public Health (Birmingham City Council) so that we can work with them to help provide better opportunities to promote active lifestyles which will help us understand activity levels and/or barriers to Moving More and Eating Well in Birmingham.
