As we work in a multi-disciplinary team (nurses, doctors, physiotherapist, mental health practitioner etc.) with team members working different days of the week and with different special skills it is not possible and/or necessary to see one particular health care professional every time.  However, we appreciate that when you are dealing with a particular health issue (e.g. cancer, chronic pain, mental ill health) or for a particular illness episode it could be important to have access to a particular doctor/other healthcare professional.

We are looking at ways of ensuring this happens (having planned reviews, having a key worker etc.)
Communication and accessing Information

We are exploring ways to extend the use of text messaging for example to ask you to make an appointment with us, to send you your blood results, share links to useful information on the internet after your consultation, share pictures of skin rashes or your blood pressure readings you took at home, care plans etc.  with the main aim to improve communication and streamline your care.
