Surrey and Borders Partnership NHS Foundation Trust (SABP) and Guildford and Waverly Clinical Commissioning Group has asked the Royal College of Paediatrics and Child Health (RCPCH) to conduct an independent review of children’s community health services in Surrey.  These are the services run by doctors, nurses and therapists (e.g. physios/speech and language therapists) who work in community clinics and special schools and diagnose and follow up for conditions such as ADHD, developmental difficulties and chromosomal disorders.

Why we want to hear from you

The Review team is very keen to hear the views of young people who have been cared for by the service and from their parents and carers, particularly if your experience is recent or within the last eighteen months.

We want to know what matters to you, so we can ensure that children continue to receive high quality, safe care. We have asked some specific questions below but please use the section at the end for any further thoughts you wish to share.

How we will use your answers

What you tell us will be read carefully. It will remain confidential to the review team and we will summarise the responses in our report for SABP so that they are anonymous - you will not be identified.  Your views will be used, along with other information collected by the review team, to inform our findings and recommendations.

Please note we are unable to respond to or resolve individual issues, if you have a complaint or question about your or your child’s care you can contact SABP directly.

Thank you for taking the time to share your views and experiences, please pass the survey onto anyone else who may wish to share their views. We will keep the survey open throughout the review but a response by Friday 30 March 2018 would be appreciated.
