Critical Worker Form 2020

From Monday 23 March 2020, schools in England will only stay open for children whose parents count as critical workers and children who are vulnerable.

We need to ask you if all adults in your household are classed critical worker's, so we know whether your child or children will need to continue to come into school.

If you do fall within one of the Critical Worker categories, the advice is still that if it is at all possible for children to be at home, then they should be. Please consider if there are any other arrangements you can make for child care during this period. The more children who are able to isolate, the more effective this measure will be in slowing the spread of Coronavirus.

Please take 2 minutes to complete this short form if you would like to be considered a critical worker. This will help us gather this information quickly and plan for the children we need to support in school next week. Please complete a separate form for each parent or carer in your household.
