This survey is being carried out on behalf of Sandwell Council and is open to all parents and carers of children aged 6 years and younger.

The council are seeking the views of local families around the subject of how they prepare their children to be ready for school at age 4 when they start reception.

We are interested in hearing from:
- Parents and carers living in Sandwell who may have a child yet to start school
- Those who may have a child who is either in Reception or Year 1 already at a school in Sandwell

Survey responses are anonymous, and the survey should take around 5 minutes to complete.

* 1. Who do you think should have the main responsibility for children having the skills and independence to start primary school? (Please select ONLY one)

* 2. Which of the following would help you and your child (or children) to get ready to start primary school?

If your child has already started primary school, what would have helped you?

* 3. Is there anything else that you feel would be useful for parents in helping to prepare their child for school?

* 4. What do you feel are the best ways that information can be shared with parents about helping their children to prepare for the start of primary school?

* 5. If you have a child who has started school already, is there anything that you feel would have helped them to be more prepared to start primary school?

* 6. If you have a child who has not started school already, is there anything that you feel would be useful to you personally in supporting them in being ready for starting school?
