Survey Introduction

Sands' Memory Boxes are an incredibly important element of the support that we offer to bereaved parents; they allow us to reach out at a moment of devastation and grief to offer the opportunity to make treasured memories.  It is vital that we make sure we are meeting the needs of bereaved parents and that the contents of the boxes are right for them.

To help ensure we are doing this, we would like to ask bereaved parents for their views on Memory Boxes; we would also like to ask for the views of other people who are a part of the parents' journey - Sands Groups, members, trustees and staff; and health care professionals.

Please could you help us by filling in this survey, so we can ensure that Sands is providing the right support for bereaved parents?

The survey will be open until 31 October 2017 and we would be enormously grateful for your support.

This survey is very thorough. It has 6 sections and will take around 15 minutes to complete.

SECTION 1: THE ACTUAL BOX
SECTION 2: CURRENT CONTENTS OF THE BOX
SECTION 3: FOR MEMORY BOX PROVIDERS
SECTION 4a: DISTRIBUTION OF THE BOXES - FOR HEALTH PROFESSIONALS ONLY
SECTION 4b: DISTRIBUTION OF THE BOXES - FOR ALL OTHER GROUPS
SECTION 5: ANY OTHER COMMENTS YOU WISH TO MAKE
SECTION 6: ABOUT YOU.

Many thanks for your help.
