Thank you for signing up to attend community organising for health training with us. The course is accredited via ABC Awards.

To ensure that your needs are catered for and that you receive a certificate on successful completion of your chosen course/courses, please complete the information below. Responses are required for all starred questions.

Additional questions asked give us information to ensure our training meets your expectations.

Certificates are provided through ABC Awards. Starting Point will provide Community Organisers Ltd with limited information to approve the certificate (your name and date of course attended) who in turn will notify the awarding organisation ABC Awards, in order for you to receive your Certificate of Achievement for accredited training .

Starting Point store all learner details securely and all diversity data is anonymised and used for analysis. Your information will be held by Starting Point for the period of time required by ABC Awards in relation to the course you have taken part in.

You have the right to access, change or ask for your data to be deleted and can withdraw consent for data sharing. You are able to lodge a complaint with the Information Commissioner’s Office should you feel it necessary to do so. Starting Point can be contacted at hello@startpoint.org.uk.
