Youth Voice is an exciting new project set up by Off the Record that allows young people to have their say on key decisions about the kind of services/activities we offer and how these are designed and delivered. As young people who have worked with Off the Record we believe that you are the best people to tell us what we’re doing well and where we can improve!

BUT before the Youth Voice project can get started we need to hear young people’s views on what they think the project should look like. Please help us by answering a few questions.

(If you need help understanding or completing any part of this questionnaire, please ask a member of staff. All information provided will be kept anonymous.)

* 1. Does this sound like a project you would like to get involved with?

* 2. There’s lots of ways young people could take part in the Youth Voice project. Which of these would appeal to you most? (You can tick as many as you like)

* 3. We hope the project will also be a chance for young people to gain valuable experience/ skills. Please tell us what kind of experience/ skills you would most like to gain through taking part in the Youth Voice project by ranking the below in order of importance (1 being most important to you, 5 being least)

* 4. How do you think we could motivate young people to become involved in the Youth Voice project?

* 5. What do you think could prevent or put young people off taking part in the Youth Voice project?

* 6. If you were to participate in the Youth Voice project how often would you prefer to meet?

* 7. Is there anything else you would like to share about what you think the Youth Voice project should look like? Please tell us!
