This information is being collected by Lloyds Bank Foundation for England and Wales and will be used to identify a potential trustee for your charity, and for programme management and monitoring purposes. Your data will be used only for the purposes specified. Your data will be kept for a period of 7 years.

The information captured in this form will be shared with Lloyds Banking Group as part of the trustee matching process.

Under the General Data Protection Regulations, you have the right:

- of access to your personal data - if anything is inaccurate or incorrect, please let us know and we will correct it (and request that Lloyds Bank Foundation for England and Wales also corrects their record);

- to have your data expunged from our database (and from Lloyds Banking Group’s database).

To be eligible to apply to participate in this programme, you must fulfil the following criteria: 

1. Be in receipt of a grant from Lloyds Bank Foundation for England and Wales

OR 

2. Be affiliated with Lloyds Bank Foundation for England & Wales or Local Motion in any of the following places: Bolsover, Carmarthen, Enfield, Great Yarmouth, Halton, Lincoln, Merthyr Tydfil, Middlesbrough, Oldham, Redcar & Cleveland, Telford & Wrekin or Torbay, with 70% of your provision being delivered in one of the localities listed above and fulfil the following two criteria:

Constituted as registered charities, community interest companies, social enterprises, or community organisation.
Have an income of less than £1 million.

If you don't meet the above criteria or are seeking a trustee with specific knowledge of an issue (e.g. housing, offending, domestic abuse) rather than completing this survey, please speak to your Regional Manager if you are currently in receipt of funding from Lloyds Bank Foundation for England and Wales - they may be able to explore alternative recruitment options. 

If you meet the above criteria, we will advertise your trustee roles to Lloyds Banking Group colleagues for a period of up to 12 months. Should you wish to retire your interest before this for any reason, let us know. Whilst we hope to make an introduction/introductions via this Programme, we cannot guarantee introductions. As such, we advise you to pursue other recruitment channels in parallel with the Trustee Programme.

Before you complete the survey

The survey asks you to include details of the skills that you are seeking in a trustee. Conducting a skills audit of your Board may help with identifying your needs. Look for the guide to recruiting, selecting and inducting trustees on our website to help with this. If you need more hands on help, please speak to your Regional Manager.
