SMI (Severe Mental Illness) Annual Physical Health Checks - Primary Care Survey

This survey is intended to assist us to understand how can we help you to improve the physical health of people with a Severe Mental Illness (SMI). 

People with an SMI have a life expectancy which is between 15-20 years less than the general population. SMI Physical Health Checks are a key way of reducing this gap and prevent avoidable health issues and premature mortality.

Currently Derby and Derbyshire's attainment of people with a SMI receiving an annual Physical Health Check (PHC) and follow up interventions is around 32%, against the NHS Long Term Plan target of 60%. 

We understand the huge work pressures you are under and have devised this brief survey to understand how we can help you deliver good practice and reduce health inequalities. It should take around 10 minutes to complete. The closing date is the 31st May 2022.

Getting support right will benefit patients and also support attainment of QOF points.
