Alopecia UK Support Groups - Attendee Feedback Form (2022/23)

Welcome and thank you for joining one of our support groups!

To help us improve what we do for the future, it is helpful if attendees of Alopecia UK support group meetings could leave some feedback. You can do this more than once at any point in the year.

The data collected in this survey is to help us evaluate the Alopecia UK Support Group Network and will be used to guide future support provision for people affected by alopecia.

The questions we ask are anonymous; however, any identifiable information should be written in the free-text responses this will be removed prior to writing any reports or dissemination.

Please note that alopecia simply means hair loss. There are many different types of alopecia. When we use the word alopecia below we mean for it to encompass all types.
