Introduction

This online survey is being carried out by the University of Suffolk, who are working with Suffolk Family Carers.

The survey will help us to understand how support provided by Suffolk Family Carers Young Carers and Young Adult Carers teams in partnership with schools, sixth forms or colleges might make a difference to young peoples lives. If you are a young person in Suffolk we would love to hear from you. 
  • The data you provide is anonymised.
  • We do not collect any information which may specifically identify you as an individual.
  • Your participation in the survey in entirely voluntary.
  • Your participation will not influence any support you may receive from Suffolk Family Carers.
  • All data collected will be used in a final report for Suffolk Family Carers to help them make the support they provide better for young people and professionals.

If you are unsure about any of the questions or how to answer the questions, please ask for help from someone else, for example a teacher, youth worker, parent or guardian. This is not a test and there are no right or wrong answers, just fill the questions as best as you can.

At the end of the survey you will be taken to a different page to enter an email address for yourself (or a parent or guardian with their permission) to enter into a prize draw to win a £50 online Amazon voucher! Your responses here will not be linked with your email address, and you don't have to enter an email address if you don't want to.
