In 2017, organisations involved in funding or delivering maternity services across Sussex and East Surrey came together with the aim of improving maternity services and support for women and their families.

As part of this work, we are seeking the experiences of women and their families/partners that have  used maternity services in Sussex and East Surrey recently.  Gathering feedback about what currently works well and what works less well, will help to guide us to improve local services, ensuring that all women and their families have access to safe and personalised maternity care.

If you or your partner have used maternity services in Sussex or East Surrey in the last three years, we would love to hear from you.  This questionnaire is open to both mothers and fathers and takes approximately 15 minutes.

All your responses are anonymous.  If you would like to contact us directly about this, or do the survey  in another format or language, please contact Dani Plowman (details below).  After completion of this survey, should you feel that it would be helpful to talk through your pregnancy/birth experiences with a professional, please contact Dani Plowman, who will be able to signpost you to an appropriate service.

With thanks to Brighton and Hove Maternity Services Liaison Committee for allowing the adaption of their 'Walk the patch' questionnaire.
