The following document is to be completed by the Chairperson, Club Secretary and all Team Managers of each Club.  It is a requirement for all Clubs who wish to participate in the League to ensure that this document is completed. 
   
This document must be submitted by all Chairperson, Club Secretaries and Team Managers before 11:59pm on 31st August 2020.
   
Clubs failing to submit the form online will incur a fine in accordance with the Fines Tariff and will not be permitted to participate in the League until properly received.

The Club will promote best practice in all aspects of participation in the Surrey County Womens & Girls League and ensure that their Officials, Players and Spectators abide by the laws of the Football Association and the Surrey County Womens & Girls League 2020-2021 Season Rules, which can be found on the League's Website here. I also understand that, any participant whose actions / behaviour warrant the view of the League Management Committee that they cannot reasonably guarantee that to be the case, will have their fixtures withdrawn until measures are in place to satisfy the League to the contrary.
